PCS To/From Guam:
- To Guam: See Guam Department of Agriculture Import Guidelines. Military personnel arriving from non-exempt areas or that do not meet the exempt from quarantine criteria will be lodged at Andersen Pet Lodge on Andersen AFB. Pets will be examined for a quarantine release examination by a military veterinarian on the first business day after arrival, mission permitting. Fees must be paid to the VTF before pets will be released from the Quarantine facility. (See PHA-Guam Memorandum – Andersen Pet Lodge).
- From Guam: Service members and eligible patrons that are departing Guam will need a health certificate from a civilian or military veterinarian. As of Aug 2020, the health certificates also need to be endorsed by Guam Department of Agriculture (DoA). Health certificate paperwork is deposited in a drop box at DoA before 1400 daily and generally has a 24 hour turnaround for endorsement. Health certificate appointments should be scheduled to occur within 10 days of the departure date, or in the case of foreign travel, within 10 days of the date your pet will arrive in that country. At a minimum, to receive a health certificate from one of PHA-Guam Veterinary Treatment Facilities, your pet must be current for core vaccines (rabies, distemper, canine adenovirus, parvo) and have a current heartworm test (within the previous 12 months), and negative fecal test within the last 6 months. Cats are required to be current for rabies, feline viral rhinotracheitis and calicivirus (FVRCP), and a negative fecal test within the past 6 months. You should notify your veterinarian as far in advance as possible when you know you will be traveling from Guam with your pet. Vaccines and diagnostic test satisfying the requirements of health certificates should be completed before presenting for a health certificate at one of the military facilities. Records for care at civilian facilities must be sent or copies provided at least three working days prior to your appointment. This avoids duplication or repetitive procedures and increases the efficiency of your appointment.
Andersen VTF Policy:
- Late/No show policy- personnel will lose the privilege of services at both military VTFs if more than three missed appointments in a 12 months period.
  - Appointments cancelled with less than 24 hours’ notice may be considered a no-show appointment.
  - Appointments will need to be rescheduled if the patient arrives more than 10 minutes late.
- Privately owned animal veterinary services are only one of our missions. At times our other missions will take precedence and we may need to reschedule appointments, sometimes on short notice.

Common over the counter preventatives. Prescription medications from military veterinary facilities require a valid doctor/patient relationship which means an examination by a military veterinarian within the previous 12 months. We cannot honor prescriptions written by veterinarians outside the military global veterinary medical practice.
